amavis sometimes throws queue file write error and produces duplicates when load is high
to fix, evaluate
  • $child_timeout
  • $smtpd_timeout
  • smtpd_proxy_timeout
  • smtp_data_done_timeout
  • smtpd_data_done_timeout

($ is amavis, without $ is Postfix)
